To self-enroll, click the "Sign up" button above and be ready with this information:

• First & last name
• Date of birth
• Email address

2. Enroll by email invitation

If MLK Community Healthcare has sent you an email invitation to the patient portal, follow the instructions to create an online account. If you haven’t received an invitation or can’t locate it, consider self-enrollment.
