To avoid delays, please arrive 15-20 minutes early to your appointment to complete registration. If you need to cancel, please let us know 24 hours in advance so that we may see other patients.

Before your appointment

About 1 week before your appointment you will receive a link via SMS or email. Click on the link and enter your information. You will receive additional reminders as the date of your appointment gets closer.

Arriving for your appointment 

You can also check-in and register at your doctor’s office by scanning the QR code posted at the primary care using your mobile phone.

To speed up your visit

Ensure you finish your registration, history, medication history, screenings, and other documents before you arrive!

When you come for a doctor's visit, please bring:

• Your driver’s license or identification card
• Your health insurance card, if you have one

 All your medications. Our care team will ensure that your prescriptions are current.

Your medications include:

• Prescription medication
• Over-the-counter medications such as aspirin or ibuprofen
• Vitamins
• Herbal supplements

At your appointment, your care team will send any needed prescriptions to the pharmacy of your choice. If you do not have a pharmacy, our office staff can help you find one close to home.

If you need any refills on prescriptions, always call your pharmacy and request them at least five days before your prescription runs out. They will notify us if you are out. Only call the office in case of emergency (you need immediate refills).

MLK Community Healthcare offers convenient telephone or video appointments with our doctors for patients age 18 or older, for appropriate services. If you are a current patient with a minor health condition, you may be eligible for a telephone appointment. Please call or text us at 424-529-6755 to schedule. When you call us, we will:

• Confirm you’ve had at least one in-person visit with us
• Schedule a one-hour window for the doctor to call you

Please bring your blood glucose meter (glucometer) to every appointment.  This is important so we can ensure your blood sugar levels are normal.

At MLK Community Medical Group we want our patients to be aware of what they are entitled to as a participant in the Disease Management Program, as well as what is expected of them while enrolled in the program.

You have the right to

1. Have information about the organization, its staff and its staff’s qualifications and any contractual relationships
2. Decline participation or dis-enroll from programs and services offered by the organization
3. Know which staff members are responsible for managing their DM services and from whom to request a change
4. Be supported by the organization to make health care decisions interactively with their practitioners
5. Have personal identifiable data and medical information kept confidential; know what entities have access to their information; know procedures used by the organization to ensure security, privacy and confidentiality
6. Be treated courteously and respectfully by the organization’s staff
7. Communicate complaints to the organization and receive instructions on how to use the complaint process, including the organization’s standards of timeliness for responding to and resolving issues of quality and complaints
8. Receive understandable information

Patient expectations are as follows

1. Follow care advice offered by MLK CMG
2. Provide MLK CMG with information necessary to carry out the services provided in the DM program
3. Notify MLK CMG and your treating provider if you decide to dis-enroll from the Disease Management Program
