To avoid delays, please arrive 15-20 minutes early to your appointment to complete registration. If you need to cancel, please let us know 24 hours in advance so that we may see other patients.
About 1 week before your appointment you will receive a link via SMS or email. Click on the link and enter your information. You will receive additional reminders as the date of your appointment gets closer.
A day or two before your appointment you will receive a text message from (424) 529-6755 and an email from donotreply@mlkch.org
You can also check-in and register at your doctor’s office by scanning the QR code posted at the primary care using your mobile phone.

At your appointment, your care team will send any needed prescriptions to the pharmacy of your choice. If you do not have a pharmacy, our office staff can help you find one close to home.
If you need any refills on prescriptions, always call your pharmacy and request them at least five days before your prescription runs out. They will notify us if you are out. Only call the office in case of emergency (you need immediate refills).
MLK Community Healthcare offers convenient telephone or video appointments with our doctors for patients age 18 or older, for appropriate services. Please bring your blood glucose meter (glucometer) to every appointment. This is important so we can ensure your blood sugar levels are normal.
All MLK Community Healthcare patients have access to a confidential, online patient portal that allows you to access your personal health information, get test results, see upcoming appointments, medications, send a message to your doctor, and more – 24 hours a day, seven days a week.
